Osteoporosis is a progressive chronic systemic metabolic disease of the skeleton or a clinical syndrome manifested in other diseases, which is characterized by a decrease in bone density, a violation of their microarchitecture and increased fragility due to impaired bone metabolism. According to the World Health Organization (WHO), osteoporosis ranks fourth among non-communicable diseases.
Bone loss occurs gradually and is often diagnosed only after a complication has already occurred—a fracture of the spine or hip. The medical and social significance of osteoporosis is determined by its consequences: fractures of the vertebrae and bones of the peripheral skeleton, which occur at a low level of injury.
Today, the real risk of developing osteoporosis is estimated at 35-40% in women and 10-12% in men. At the same time, only 10% of patients know that they have osteoporosis – most often due to the fact that the disease has already manifested itself in spontaneous fractures.
Classification of risk factors for osteoporosis according to the International Osteoporosis Foundation (IOF): endocrinological, genetic, as well as those caused by lifestyle (overweight, alcohol, smoking), concomitant pathology and long-term use of certain medications.
The generally accepted definition of osteoporosis involves the presence of several not always related characteristics: decreased bone mass, abnormal bone microarchitecture, and a high risk of fractures. Traditionally, the only indicator of bone strength and fracture resistance is the degree of skeletal bone mineral density.
DIAGNOSIS OF OSTEOPOROSIS
The only method that allows you to confirm the diagnosis of osteoporosis and identify the disease in the early stages is bone densitometry, which, according to WHO, is the “gold standard” in the diagnosis of osteoporosis. Densitometry is capable of recording minimal loss of bone mass, 2-5%, and assessing the dynamics of bone tissue density during the development of the disease or the effectiveness of treatment. It is important to know that the level of calcium in the blood does not characterize the condition of bone tissue. Very often, the normal level of calcium in the blood is maintained only because it is “washed out” from the bone tissue.

Laboratory diagnostics of patients with osteoporosis is aimed at identifying risk groups for osteoporosis, conducting differential diagnosis with other diseases of the skeletal system, determining biochemical markers of bone formation and resorption, identifying causative factors of secondary osteoporosis, assessing methods of pharmacotherapy of the disease, etc.
OSTEOPOROSIS THERAPY
In The «AltMed» Med. Center conducts a comprehensive treatment and health program “Stop Osteoporosis!” — a group of specialists (endocrinologist, rheumatologist, dietician-nutritionist, therapist, gastroenterologist, physiotherapist, reflexologist, psychologist, and kinesiotherapist) carries out comprehensive diagnosis and treatment of osteopenia and osteoporosis using medications (chondrotropic and osteotropic peptides that stimulate the digestive system), diet therapy (prescribing a therapeutic nutrition system, as well as complexes containing vitamins and microelements that compensate for their deficiency in the body) and non-drug (physiotherapy, reflexology, kinesiotherapy and psychotherapy) methods for the prevention of bone fractures and the treatment of pain syndromes.
Treatment should be aimed at both relieving pain and preventing the progression of osteoporosis, and, therefore, preventing subsequent fractures. Currently, bisphosphonates lead the drug market for the treatment of bone osteoporosis. Calcium and vitamin D supplements are used in addition to other treatments. Hormone replacement therapy with estrogens and protestogens prevents bone loss in postmenopause and reduces the risk of fractures.
Physical exercise is a mandatory component of any program for the management of patients with osteoporosis of the spine, both at the preclinical stage (osteoporosis according to densitometry without fractures), and at the stage of fractures or deformities. Exercise helps strengthen muscles and maintain bone mineral density. Massage is used no earlier than after 3 – 6 months. The effectiveness of the treatment is monitored by repeated measurements of bone mineral density, as well as determination of biochemical markers.
PREVENTION OF OSTEOPOROSIS
Prevention and treatment of osteoporosis include: non-drug treatments and pharmacotherapy. Prevention and treatment of osteoporosis can be divided into 3 stages:
- the first is primary prevention, aimed at achieving peak bone mass during maturation;
- the second is secondary prevention at the stage of physiological bone loss;
- the third is the treatment of osteoporosis and its complications.
3 principles of prevention and treatment of osteoporosis:
- the first – etiological (treatment of the underlying disease in secondary osteoporosis or correction or withdrawal of “iatrogenic” drugs for osteoporosis);
- the second is symptomatic (use of a diet balanced in calcium salts, phosphorus, protein, taking calcium salts, dosed physical activity, physical therapy, for severe osteoporosis – orthopedic treatment, painkillers and muscle relaxants);
- the third is pathogenetic (basic principle), aimed at normalizing the processes of bone remodeling.
All means of prevention and pathogenetic therapy of osteoporosis can be divided into three groups:
- first – means that inhibit bone resorption;
- the second is drugs that stimulate bone formation;
- the third is drugs with multifaceted action.
Physical activity helps strengthen bones. Over the past 30 years, the idea that mechanical load affects the strength and size of bone has been developed and confirmed. Prevention of osteoporosis includes: giving up bad habits (smoking, alcohol, coffee, etc.); nutrients necessary to restore lost bone mass; physical exercise (regular walking, swimming, exercise equipment), etc.
